If you experience difficulty with the online application, email us at info@yogichuck.com and we will send you a fillable PDF application to complete.
Once you have submitted your application, you will receive a confirmation email, followed by a copy of your new policy declarations and certificates of insurance.

Pricing and coverage for individual instructors:
Standard Total Premium: $136.00 a year
Higher limites of insurance coverage can also be purchased:

The Instructors Policy includes:
- general liability (i.e. - slip and fall)
- professional liability (i.e. - improper adjustments, advice, etc)
- sexual harassment/molestation
Instructors Policy coverage:
- $500,000 each occurrence
- $500,000 personal/advertising injury
- $100,000 fire legal liability
- $1,500,000 general aggregate
- $500,000 products/completed operations
- $5,000 medical payments
- Professional liability included
- $100,000 abuse/molestation per occurrence
- $100,000 abuse/molestation aggregate

Adding Additional Instructors to Your Policy
If you need and additional insured listed on your policy, please provide the name, address and contact information for each additional insured on your applicatioin or e-mail us your request info@yogichuck.com. Our new program allows for up to 4 Free Additional Insureds on your policy!
